2012 Registration Forms
The LINK at the end of this page contains all the required forms to register for the 2012
season.

Please download all the forms, fill them out completely and return all forms and Club fee
to Coach Mike Morel at 33940 Country View Lane, Solon, OH 44139.

Please note the following:
  1. Return ALL forms and a copy of the US Lacrosse confirmation email to Coach Morel
  2. Players may not participate in any practices or games until all forms are submitted.  
  3. All players are required to join US Lacrosse.  The fee to join US Lacrosse is $25.  All
    players must self-register at www.uslacrosse.org.      
  4. The fee is $225 made payable to Solon Attack Youth Lacrosse Club.
  5. Parents and players please carefully read the Parent and Player Codes of Conduct.  
    The team makes every effort to have players, coaches and parents follow these
    principles.
  6. The Solon Attack strongly discourages the playing of two sports at the Middle School
    (7th and 8th Grade) level starting at the beginning of the outdoor season in April
    2012 and for the remainder of the lacrosse season.  
  7. Over the years of fielding the lacrosse team, the coaches have found that playing
    more than one sport during the spring season detracts from the ability of the player
    to learn and put into practice the fundamentals of either sport.

All Solon Attack Middle School lacrosse players will be expected to attend all scheduled
practices and games.  The only acceptable excuses for missing any of these activities are
illness, school obligations or a family emergency.  

The one-spring-sport policy does not apply to the Youth (5th & 6th grade) lacrosse program.
